What to do when a member receives a notice of uncashed check?

If a member has received a notice of uncashed check affidavit and brings it into a branch, then a Kace ticket needs to be opened using the “back office” option. The task category to select is “Declaration of Loss”. 

Scan and attach the Notice of Uncashed Affidavit to the Kace ticket. 

They can also mail it to the Milwaukie Branch location if they are unable to bring it in.
