Find the member's account by name stated on the left hand side of the Deed of Trust.

While in the member's account, click on account balances icon and find the suffix of the HELOC or Home Equity loan

On the left hand side of the screen, click on Main Menu and select System Operations, then click on Reports.

While under Reports, click on Loans. In the middle of the screen, scroll and find Cover-5-Deed of Trust 020 and double click.

In the middle of the screen under Input Parameters, in Account Select box enter the member's 6 digit account number as well the check digit number. (Ex. 123456-7)   In Suffix box, enter the HELOC or Home Equity loan suffix number.

In the upper left hand corner of the screen, click Execute as it will generate the cover sheet. (may take a moment to generate)

On the upper right hand corner of screen, click on the i icon then select on the newly generated cover sheet and print it. Attach cover sheet over Deed of Trust documents and scan it to Laserfische.
